Nagpur: A 23-year-old young man was admitted in the early hours of the morning with accidental trauma. The poor labourer was working at a construction site to demolish a wall in the industrial unit which collapsed while he fell from a height. He was impaled on the left side of the chest wall with an iron rod and suffered severe injuries in the abdomen.
The critically injured patient was immediately resuscitated in the emergency room and trauma protocol was followed. According to the reports, the patient had pneumothorax and splenic injury. A pneumothorax is a condition in which air leaks between the lung wall and the chest. The patient showed symptoms of shortness of breath as the lungs were compressed by the air and leaking blood. The patient also had severe blood loss due to a serious injury to the spleen and. Spleen is a internal organ with rich blood flow and this injury leads to hypovolemic shock as there is severe blood loss.
Dr. Dhananjay Kane, Senior Consultant general surgeon attended to the patient and performed a three-hour long laparotomy, splenectomy, and diaphragm repair. The patient was electively connected to a ventilator and extubated the next day. The patient’s condition steadily improved. RT and Foley were removed. Shortly thereafter, oral nutrition was started and eventually, the drain was removed.
Patient was under the care of Senior Critical Care Physician Dr. Rajesh Atal and team comprising of dedicated 24×7 intensivist covering Dr. Mohit Gharpure and Dr. Akshay Burlawar. Dr. Smita Harkare, Senior Anaesthetist and supporting staff provided exceptional surgical cover in OT. Radiologists Dr. Amol Gawale, Dr. Mayur Mahadule, and pathologist Dr. Shailendra Mundhada offered vital diagnostic support. At the same time, physiotherapist Dr. Atul Dekate and dietician Ms. Dipti Hatwar provided rehabilitation and nutritional support. Exemplary nursing care was provided under the leadership of Sr. Priti Joseph in CCU.
On discharge, the patient performed all activities of daily living independently and showed no dependency. He left the hospital on foot.
