A 51-year-old, beneficiary of Maharashtra Police Kutumb Arogya Yojna was admitted with a history of road traffic accident.The patient was transferred to Orange City Hospital from Chandrapur after couple of days of getting treatment.
At the time of admission, the patient had paralysis neck down due to Cervical trauma (neck) which caused spinal cord compression. The CT scan of the neck revealed a peridiscal osteophyte which is bony growth along the edges.
A thorough clinical examination revealed loss of reflexes in all four limbs.With such deranged symptoms, the patient was admitted to the intensive care unit under the Neurosurgical Trauma Team consisting of Neurosurgeon, Dr. Palak Jaiswal. Medical management was under the expertise of Senior Critical Care Physician, Dr. Rajesh Atal.
On admission, the relatives were informed of the poor prognosis. Initially, the patient was in a state of spinal shock and low blood pressure which required support and stabilization before any intervention could be done. After a week, patient was medically in a condition to undergo the high-risk procedure of cervical laminectomy with anterior cervical discectomy and fusion was performed under general anaesthesia. The Anesthetic team comprises of Dr. Smita Harkare, Dr. Neeta Deshpande and Dr. Anita Pande.
Postoperatively, the patient had to be artificially ventilated and thus the chest physician Dr. Sushant Muley was involved for the specialty.
Intensivist Cover provided by a dedicated team Dr. Mohit Gharpure, Dr. Akshay Burlawar, and Dr. Suchita Naik. OT, CCU and Ward nursing & paramedical team led by Sr. Preeti Joeseph and Sr. Nisha Ashok.
Although the patient was weaned from the ventilator, a tracheostomy was required to improve breathing and remove tracheal secretions. To satisfy the need for communication, a special tracheostomy tube (with a voice valve) was inserted which would be removed on follow up visit. Once the patient was able to communicate, care became easier and more comfortable.
Dr. Usha Nair, Managing Director mentioned that for an extended stay of one month in the hospital for a patient it is of utmost importance to follow the infection control practices as per the protocols so the patient does not succumb to hospital aquired infection. Apart from the above multidisciplinary team of doctors, patient also received comprehensive care provided by Dr. Amol Gawale, Dr. Mayur Mahadule, Radiologists, and Dr. Shailendra Mundhada, Pathologist, extended the necessary diagnostic support. Rehabilitation support was given by Dr. Atul Dekate, Physiotherapist while nutrition support was received by Ms. Dipti Hatwar, Dietician.
